Disclosing and Documenting a Disability to support a request for Accommodation(s)

Step One: Log onto Accommodate to complete and submit the "Disclosure Form for New Accommodations." Incoming First-Year students should complete this step by May 29, 2015.

Step Two: Submit appropriate documentation of your disability. For more specific guidelines on documentation, please refer to the Guidelines for Documenting a Disability, and select the disability (or disabilities) that you are requesting accommodations for.

Documentation and requests should be submitted to Marni Jones, Director of Disability Services, by any one of the following means:

Step Three: Director Marni Jones will review your documentation to determine eligibility for any accommodations. Please note that sufficient time is required to review all documentation. It is possible that she will request additional documentation, or seek a consultation with an evaluator before a decision can be made. 

Step Four: Students requesting a Housing Accommodation or Dietary  Accommodation will additionally need to have their care providers complete the required documentation forms available on those respective pages.
