Financial Aid Forms
The Office of Financial Aid at Dickinson College may request additional information and/or clarification throughout the financial aid application process. To better assist students and their families, the forms below can be used in these specific situations. Some of these will be required to complete your financial aid application at Dickinson College, others are only needed if requested by our office.

Federal Verification Forms
These forms may be required in order for the Office of Financial Aid at Dickinson College to complete the federal verification process.
Verification is the process under which an applicant's FAFSA information is selected by the Department of Education or the institution (college) to ensure the information is correct.
Dependent Verification Worksheet
Your Free Application for Federal Student Aid (FAFSA) was selected by the Department of Education for review in a process called Verification. Federal law requires the Office of Financial Aid to confirm the information you and your parents reported on your FAFSA before awarding federal student aid. To verify that you provided correct information, we will compare your FAFSA with the information on this worksheet and any other required documents. We may ask for additional information. If there are differences, your FAFSA may need to be corrected.
You and at least one parent must complete and sign this worksheet, attach any required documents, and submit the materials to the Office of Financial Aid. If you have questions about Verification, contact our office as soon as possible so your financial aid will not be delayed.

Independent Verification Worksheet

Institutional Financial Aid Forms
These forms are usually requested on a case-by-case basis by the Office of Financial Aid in order to get additional information or resolve conflicting information. Usually this happens after we meet with a student and their family, or after we review their FAFSA and CSS Profile.
Asset Form (Student)
Our office may request this form if we need additional details on the assets listed on the FAFSA and/or CSS Profile. Assets may include investment accounts, 529 accounts, business/farm worth, trust funds, other real estate, and cash, checking, and savings accounts.
Asset Form (Parent)
Our office may request this form if we need additional details on the assets listed on the FAFSA and/or CSS Profile. Assets may include investment accounts, 529 accounts, business/farm worth, trust funds, other real estate, and cash, checking, and savings accounts.
Family Grid Form
This form may be required if there is a discrepancy in the family size reported on the FAFSA and CSS Profile.
Non-Filer Statement and Earned Income (Student)
If you (student) did not file taxes in the FAFSA base year (prior-prior year), you may be required to complete this form in order to verify your income for that year.

Off-Campus Study Abroad (Non-Dickinson Program)
Students must complete this form, so our office can process their federal and/or state financial aid while they are studying abroad in a non-Dickinson program. Students in non-Dickinson programs are not eligible for Dickinson or Scholarships, FSEOG, Hurwitz Loans, and Federal Work-Study assistance.

By completing this form and submitting into our office, you give consent for our office to disclose your educational and financial records with the individual(s) named on the form.
SAP Appeal Form
This form is required if you fail to meet Satisfactory Academic Progress (SAP) and appeal the decision due to extenuating circumstances.
