Registrar Office Forms

In an effort to make submission of forms easier for students and faculty, the Registrar’s Office is working with LIS to transition paper/PDF forms to a completely online process.

Students – For CLIQ forms, log into the Gateway, click on the CLIQ icon, then choose Registrar’s Office Forms on the menu.  BEFORE SUBMITTING ANY CLIQ FORMS, be sure to CONSULT WITH EACH PROFESSOR who you include on the form. Once submitted, you will receive an email and so will the first professor, (ie. advisor or instructor). Once that professor completes their part, the next professor (ie. new advisor or department chair), if needed, will receive an email. When all approvals are received, the Registrar’s Office will be notified to update your record. When complete, you will receive a final email. You can check the status of these forms on CLIQ at any time.

Faculty – For CLIQ forms, when students submit a form, you will receive an email indicating you need to log into CLIQ to complete your approval. Instructions are included on the CLIQ screen. When your part of the approval is complete, the student's form will disappear from your CLIQ screen. (Note: you may need to refresh the page).

Grade Change Form CLIQ - Professor's class roster Choose Grade Change from the drop-down beside the student's name. Guidelines on Grade Changes.
Incomplete Grade Report Form CLIQ -Professor's class roster The student should contact the professor about an incomplete (based on illness or other serious emergency). Then the professor will access their CLIQ class roster and choose Incomplete Grade Report from the drop-down beside the student’s name. Here are the specific Guidelines on Incomplete Grades.

Letters of Recommendation or Reference Release Form For those with Gateway access, use the online form via this link.

Overload Registration Form CLIQ - Registrar's Office Forms If a student has requested both overloads, they must petition ASC. please see this webpage for more information. The deadline for submitting Overload forms if 4 pm Friday of the Add/Drop period.
Pass/Fail Request Form CLIQ - Registrar's Office Forms The deadline for submitting pass/fail forms is 4 pm Friday of the Add/Drop period.
