Benefit Forms
The Flexcomp and Retirement Election Form, Life Insurance Enrollment Form and other applicable insurance forms MUST be returned to Human Resource Services within the first 31 days of your employment.
- Flexcomp and Retirement Election Form 2014-15 - This form is mandatory and MUST be returned to Human Resource Services within the first 31 days of employment.
- Life Insurance Enrollment and Beneficiary Form- This form is also mandatory. Life insurance is provided by the College at two times your annual salary, with reductions made based on age.
- Insurance Enrollment/Change Form - Necessary to enroll yourself and your dependents (if applicable) to the health, dental and/or vision coverage.
- 2014-15 Insurance rates (July 1, 2014 - June 30, 2015)
- Supplemental Accidental Death & Dismemberment Enrollment Form - Necessary to enroll yourself and your dependents (if applicable) to the supplemental plan with The Hartford.
- To begin making contributions into a retirement account, indicate the per pay or annual amount on the above Flexcomp and Retirement Election form and complete the enrollment process. Annual limits are established each year by the IRS.
