Updating your HR & Benefits Information
The Life Status Change form and the completed appropriate Insurance Enrollment/Change form, are used to notify Human Resource Services of Life Status Changes within 31 days of the event and related benefits changes. Click the link to access forms.
Flexible Spending Accounts

Lactation Policy
