Health Forms and Policies
Dickinson College requires that incoming students comply with the Pre-Admission Health Policy as listed below:
Complete the medical/online health history form through the Student Forms tab in Gateway.
Schedule a physical exam with your health care provider.
Submit the medical record form, completed and signed by your health care provider by July 8, 2011.
Students who have not completed the immunization requirements will not be able to directly check into their housing assignment upon arrival to campus.
All medical forms are available by clicking on the links below. If you cannot retrieve them online, please contact the Health Center by phone (717-245-1835) or e-mail (health@dickinson.edu) and we will send them to you.
