Meal Plan Accommodations

 Request for Meal Plan Accommodation Based on a Documented Disability or Health Condition

All students living on campus are required to purchase a meal plan. Students with special medically-necessary dietary needs may require an accommodation to the meal plan. Requests for meal plan accommodations are to be made by following the Process for Requesting a Meal Plan Accommodation.

Students seeking accommodations to the meal plan will need to submit the appropriate documentation to the Director of Learning Skills and Disability Services. Documentation must include:

1. Disability Disclosure Form (if your disability or medical condition is not listed, check "other" and specify)
2.  Meal Plan Accommodation Request Form - to be completed by the student
3.  Meal Plan Accommodation Documentation Form - to be completed by the student's health care provider and sent
4. Documentation of the disability

Meal Plan Accommodation Request Deadline:

• Meal plan accommodation requests for the 2012-2013 school year should be submitted by July 31, 2012.
