Privacy Statement for Students
The Dickinson College Wellness Center is committed to keeping your protected health information (PHI) confidential. The only PHI collected about you while participating in this clinic is listed on the 2018 Flu Clinic Participation Form filled out by you. The Wellness Center staff member will also add information about the vaccine and injection site to this form. This form will be scanned into your electronic health record maintained by the Wellness Center to ensure privacy of this information. This information will be kept in accordance with the Wellness Center privacy policy, accessible here. The flu vaccine date will be added to your immunization record so that it is available on the Patient Portal. You can then download and print copies for yourself and forward this information to whomever you wish.
