ANSWERS TO MOST FREQUENTLY ASKED QUESTIONS

 

1. Q: I have lost or misplaced my the Health Forms, how can I get more?

 

A: You may click here on Health Forms to print new ones. If you are unable to print the form, please e-mail "health@dickinson.edu" to request forms be sent to you. Please remember to include your full name, address and phone number along with your request.

 

2.     Q:  Is it ok if my health forms are late?
 

       A:  We would very much like your health forms returned by the deadline. If however there are circumstances which make that impossible, we ask the forms be returned as soon as possible.
 

3.     Q:  I can’t find my immunization records, what should I do?
 

A:    If you cannot find dates of immunizations, and your doctor or high school does not have your records on file, you will need to get a blood test (called titers) to verify you have immunity to measles, mumps, and rubella. This blood test would have to be ordered by your health care provider.
 

4.    Q:  I was told we would be getting information about the Student Health Insurance.  When will it be sent?
 

      A:  The College sponsored student health insurance brochure is available for you to view at Student Health Insurance Brochure. All accounts will be billed for the premium through the Student Accounts Office. If you have health insurance coverage which will reimburse you for medical care while you are away from home, and therefore you do not want to purchase the College sponsored insurance, you must complete a waiver form. This waiver form is to be completed on-line. You will find this form by either going to the Student Accounts home page or clicking here for the Waiver Form.   If you are unable to complete the form on-line, please contact the Student Accounts Office at 717-245-1657 or e-mail "stuaccts@dickinson.edu", for a hard copy to be sent to you.   

 

5.     Q:  My son/daughter needs an air conditioner because of allergies. How do we get one?

A. The students health care provider must send a written verification that an air conditioner is required for medical reasons. Documentation should be faxed to the Health Center at 717-245-1938, or mailed to:

6.     Q:   Does it cost anything to be seen at the Health Center?
 

      A:  There are small charges for medical lab tests and prescription medication. For a complete list, please go to our "Fees" page.

       If you are a recent graduate and need immunization information sent to you for graduate school, or employment,  please e-mail your request to "health@dickinson.edu" or fax to 717-245-1938. Your request should have the following information: name, date of birth, date of graduation, address where information is to be sent and telephone number where you can be reached. Please state exactly what information you would like, for instatance, immunization information only, or the entire record. Copying fees may apply. If you transferred or withdrew, please let us know, alone with the year you left Dickinson.
