Supervisor Forms

New Hire Forms:

  • Payroll Contract (PDF format) - This form must be filled out by the supervisor and signed by both the student and the supervisor. The completed form must be submitted to the Student Employment Office before starting employment.
  • I-9 form- Employers are responsible for completing the I-9. You must complete Section 2 by examining evidence of identity and employment eligibility within three (3) business days of the date employment begins. The completed form must be submitted to the Student Employment office before the student can start employment.
  • W4 form (PDF format) - This form must be completed and signed by the student. The completed form must be submitted to the Student Employment Office before starting employment.
  • Borough of Carlisle Certificate of Residence (PDF format) form- This form must be completed and signed by the student. The completed form must be submitted to the Student Employment Office before starting employment.
  • Local Service Tax Exemption Form- this is a new form. If a student makes less than $12,000 per year, they will be exempt from paying the $52 Local Services Tax.
  • Direct Deposit form- This form must be completed and signed by the student in order to particpate in direct deposit. Students should submit the completed form to the Student Employment Office before starting employment. NOTE:   THE STUDENT'S FIRST PAY WILL BE IN THE FORM OF A CHECK
  • Exemption Approval Form (PDF format)-This form is required when a department hires a First Year Student. This form is to be used after the hiring department has requested an exemption, and they have been granted either a Conditional or Unconditional Exemption approval.
  • Performance Evaluation Form-This form allows supervisors to formally evaluate a student’s work performance at least once per year.
