Paperwork

There are several documents which need to be completed before starting employment at Dickinson. You may be familiar with some of them already.

Please note: Adobe® Acrobat® reader (version 3.0 or greater) is required to view and print some the forms.

1. A student employment form (payroll contract) is required. It is an agreement between the student and the hiring department. The contract states the length of time you will be employed, and the rate you will be paid.
2. The I-9 form is required by the US Citizenship and Immigration Services Department as proof of work eligibility in the United States. Identification is required to complete this form. A list of acceptable identification documents is on the back of the I-9 form.
3. The Internal Revenue Service requires a W-4 form. The manner in which this form is completed determines the amount of federal tax withheld from your pay.
4. The Borough of Carlisle requires a Certificate of Residence form to be completed for tax tracking purposes. Please see the form for additional information and instructions.
5. Local Service Tax Exemption Form- this is a new form. If a student makes less than $12,000 per year, they will be exempt from paying the $52 Local Services Tax.
6. For direct deposit, each student must complete a direct deposit form, which contains bank information. The Student Payroll office will need bank account information and ABA (routing number) for the direct deposit. You can arrange for Direct Deposit to your home bank, or to a local account. It is not necessary for students to open a new account in the Carlisle area. NOTE:   YOUR FIRST PAY WILL BE IN THE FORM OF A CHECK
6. Additional paperwork may be required for non-resident aliens. Please contact the Office of Global Education for more information.

All forms are available from the Student Employment Office, located in the Biddle House, or from the Staff Associate in the hiring department. Assistance in completing the forms is also available.
